Please make sure that ALL forms are completed, and all the necessary items are included!  
ALL TRAVEL PLAYERS MUST HAVE A NEW PICTURE, SIGNATURE, AND A BIRTH CERTIFICATE ON FILE AS WELL AS ALL NEW FORMS FOR THE FALL SEASON.   

New this year -

**ALL travel players pay $15 uniform fee for the new uniforms. This is a YEARLY expense now. You will receive a NEW uniform every year starting this fall.

**Confidential Financial Assistance is available. Contact Bob Stum IN WRITING for assistance. Your request and information will be kept private.

Registration fees this season:
**All ages as of July 31, 2009!!**
Attention Recreational Players: If you choose the wrong age group, we will make it correct in the computer system.  If you want your player to move up an age group, we MUST be made aware with a note included on the registration form.  Please do not just "check" the next age group's box or we will assume the wrong age group was chosen.

Recreational:
U6 (5 yrs old) - $35.00
U8 (6 & 7 yrs old) - $45.00
U10 (8 & 9 yrs old) - $45.00
U16 Rec (10 - 15 yrs old) - $45.00

Travel:
U10 - U14 Boys (8 - 13 yrs old) $65.00 registration PLUS $15.00 uniform fee.
U10 - U18 Girls (8 - 17 yrs old) $65.00 registration PLUS $15.00 uniform fee.
